you need to go to this site to apply for the benefit

once you qualify.. you will get a code
you take this code and go to your provider and fill out their ACP program paperwork with the code
QUALIFICATION FOR THE PROGRAM
Participates in one of these assistance programs:
The National School Lunch Program or the School Breakfast Program, including through the USDA Community Eligibility Provision;
SNAP
Medicaid
Federal Public Housing Assistance
Supplemental Security Income (SSI)
WIC
Veterans Pension or Survivor Benefits
or Lifeline;
Participates in one of these assistance programs and lives on Qualifying Tribal lands:
Bureau of Indian Affairs General Assistance

1000 minutes of voice calling
3G data speeds or better
2GB data usage
For broadband internet: at least 15 Mbps download speed and 2 Mbps upload speed

I know some home owners who take advantage of all WI rebate/discount/"free" programs. Wish I'd known sooner. This past year WI had a discount for a digital thermostat I wanted but wasn't willing to spend $149.99 on. They bought one for $60.
